complete the Golf Contest Insurance agreement. Please download the
Coverage Agreement
by clicking on the "Download Coverage Contract" button to the right.
Please print the agreement, complete the form and fax it to 215-322-7678.
A sales representative will contact you within 24 hours of receiving the
completed form. |

our Putting Contest Coverage Agreement to receive an insurance premium estimate. For
us to supply you a
quote, we will need to know the following:

- Distance of Putt (40ft or 50ft)

| Complete a Contract: |
Using the price quote you received from Global Golf
Events, complete the blank contract you received with your coverage estimate.
Submit the contract for processing. |
| Receive Your Prize Package: |
After receiving your contract, a representative will
call you using the phone number you provided to confirm receipt of your
contract. At that time, we will begin printing up your event signage,
and secure the appropriate coverage for your event. We will then ship
you your signage via FedEx, and mail you a Certificate of Coverage. |
| Let the Fun Begin: |
On the day of your event, place the signs out on
the course. If a golfer successfully sinks a putt, notify us,
and we will promptly award the appropriate prize to the winner. |
